Job Description

Reputed Home Health Care Company based in Jeddah, URGENTLY requires Patient Services Coordinator, female candidates of Saudi Nationality.

Patient Services Coordinator is responsible for facilitating care coordination services for patients and ensures to provide timely, high quality and efficient health care and support services. Develop multi-disciplinary care plan, self-management goals and referrals to both internal and external resources. Coordinates between the client or client family and insurance company, homecare health providers to meet the client needs.

Duties and Responsibilities:
-Consult with patients and family members to discuss their health problems.
-Consult and collaborate with other health care providers and specialists to set up patient appointments and treatment plans.
-Treat patients with empathy and respect and conduct oneself in a professional manner.
-Comply with organizational guidelines and health care laws and regulations.
-Follow-up with patients post home health care services.
-Under the direction of the practitioners, communicate test results and care plans to patients/families.
-Work with patients both in person and over the phone to remind and review their plan of care and progress towards their goals.
-Working collaboratively with other team members, provide care management services for home health care patients.
-Collaborate with other members of Medical Home for care coordination and care management activities including implementation of Care Plan.
-Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns.
-Assist patients in problem solving potential issues related to the home health care system
-Assist with data collection and generation of patient registry reports.
-Ensure timely documentation in Patient Medical Records.
-Participate in Performance Improvement/Continuous Quality Improvement activities, as assigned.
-Check-in on the patient regularly and evaluate and document their progress.
-Assist the care team with developing and assessing their health interventions.

Experience:
Minimum of two (2) years' experience in care coordination or clinical practices.

Education:
Diploma/Bachelor in administration or related field.
